Order Documentation and Physician Signature

Don’t let the absence of your signature be the missing link in a properly submitted order!

Pshutterstock_116032111lease be reminded that only tests that are necessary for the diagnosis and care of the patient should be ordered.

You, as the ordering physician, are responsible for ensuring that all tests are medically reasonable.  We ask you to SIGN each submitted requisition for that purpose.

There has been an increase in audits under the Medicare system (Comprehensive Error Rate Testing – “CERT”) looking for incomplete or missing documentation.

Your signature on the requisition can help ensure that the order is authenticated by the ordering physician and properly documented.

Please be sure to SIGN each submitted requisition.
